A 41-year-old man presented with fevers, weakness, dizziness, and nausea. He had hypotension (87/53 mm Hg), tachycardia, and lactic acidosis. Computed tomography was performed (see images). The patient has a past medical history of diabetes mellitus type 2 and a hospital admission 2 months earlier for a left perinephric abscess status post drainage by an interventional radiologist. After minimal output and clinical improvement, the drain was removed and he was discharged. Cultures at that time grew Klebsiella pneumoniae.
