Do you use neoadjuvant chemo prior to radical cystectomy?

A study published recently in BJU International demonstrated that neoadjuvant chemotherapy (NAC) does not increase the risk of perioperative complications in patients undergoing radical cystectomy for muscle-invasive bladder cancer.

The researchers noted that NAC prior to radical cystectomy is underused despite evidence showing that this is associated with improved survival, and one of the reasons may be clinician concern about increasing the risk of perioperative complications.

Their investigation, which included 878 patients with MIBC, found no increase in the likelihood of any complication or of re-operations, wound infection, and wound dehiscence.
