Botulinum toxin type A (BTX-A) injections may be a promising treatment for BPH patients with urinary retention who are not candidates for surgery.
Portuguese investigators studied 13 BPH patients (median age, 80 years) who had an indwelling catheter for at least three months and who were unfit for surgery because of poor general condition. Under transrectal ultrasound guidance, the patients received a 200 U BTX-A injection in the transition zone.
At two months, 11 patients (85%) had spontaneous voiding with a mean maximum urinary flow rate of 10.3 mL/sec. Prostate volume was significantly decreased from 70.5 mL at baseline to 46.4 mL at two months.
