Antihypertensive treatment with perindopril prevents recurrent stroke in CKD patients across a wide spectrum of BP levels, according to Australian and Japanese researchers. They saw no evidence for a higher risk of stroke among individuals with low BP.

An analysis of data from the Perindopril Against Recurrent Stroke Study (PROGRESS), a randomized, placebo-controlled trial showing that BP lowering with the drug reduced the risk of stroke in 6,105 subjects with prior cerebovascular disease. The investigators estimated the effects of therapy on the risk of recurrent stroke in 1,757 subjects with stage 3 or greater CKD according to baseline BP and the relationship between achieved follow-up BP and stroke risk.

 

The age- and gender-adjusted incidence of stroke increased significantly in a log-linear relationship for achieved systolic BP levels and strokes per 1,000 person-years. The association persisted after adjusting for potential confounders.
