Vascular Calcification Linked to Lower Phosphate Clearance
ATLANTA—Lower weekly phosphate clearance in patients on peritoneal dialysis (PD) is associated with increased vascular calcification, according to findings presented at the American Society of Nephrology's Kidney Week 2013.
In a study of 47 PD patients, Juan Carlos Ramirez-Sandoval, MD, and colleagues at the National Medical Science and Nutrition Institute Salvador Zubirán in Mexico City observed negative correlations between vascular calcification scores determined by radiographic studies and weekly total phosphate clearance in the hands and pelvis or in the abdominal lateral column. The scores correlated with age, time on dialysis, body mass index, and residual renal function (RRF). In adjusted analyses, weekly total phosphate clearance, age, and RRF remained independently associated with vascular calcification scores.
The researchers noted that vascular calcification contributes to increased cardiovascular risk in dialysis patients and is frequently associated with hyperphosphatemia.
The investigators measured phosphate in 24-hour dialysate. For patients with RRF, they determined phosphate clearance using 24-hour urine collection.
